ACCS can provide Community nursing service to support you with complex health needs in your home. This service aims to assist you in restoring or maintaining your health and independence at home and assist you to avoid early admittance to hospital or residential care. We can assist with medication, wound care, hygiene and help with showering or dressing. A registered nurse will assess and consult with you to determine your needs for the type and frequency of nursing and/or personal care services. To ensure you receive appropriate care for your clinical needs, the service will be closely monitored and reviewed by a registered nurse. ACCS can deliver:

- Enteral (Naso-Gastric Tube – Jejunum or Duodenum) Feeding and Management
- Tracheostomy Management
- Urinary Catheter Management (In-dwelling Urinary Catheter, In-out Catheter, Suprapubic Catheter)

- Subcutaneous Injections
- Complex Wound Management
- Diabetes Management
- Seizure Management
- Stoma Care
- Mealtime Preparation and Delivery
- Day-to-day management of medications
